When you check in, you will be asked to complete patient registration forms. The receptionist will scan a copy of your photo ID and insurance card(s) into your medical record, collect your copayment and check for a referral if one is required by your insurance company.
Please bring a list of medications and allergies, along with any X-rays, test results or other information related to the reason for your visit
